Parents, please choose the appropriate baptism status of your child and follow the link for instructions.

    • never baptized
      or
    • baptized non-Catholic (with valid baptism) but Catholic parent has not done the Catholic Profession of Faith on behalf of the child

2. CLICK HERE if your child was baptized non-Catholic (with valid baptism) and Catholic parent has done the
    Catholic Profession of Faith on behalf of the child

3. CLICK HERE if your child was baptized Catholic
